On April 30, 1986, Canadian society welcomed Caroline Slezak Kawa into the world. She eventually became “Care Failure,” the lead singer of the Toronto, Canada-based alternative rock band Die Mannequin.
Having opened for such acts as Buckcherry, Guns N’ Roses, Marilyn Manson, and Sum 41, Die Mannequin has toured Canada on many occasions. Many European tours have been completed as headliners and successful support acts for Danko Jones in 2008.
“Care Failure” has collaborated with Big Dirty Band in addition to his work with Die Mannequin, of which he was a co-founder. Caroline released her band’s CD “Unicorn Streak” in 2007; it features 10 songs and she also had an appearance in the film “Hard Core Logo 2.”

On Facebook, Nathan Clearihue broke the sad news of the loss of Die Mannequin’s Care Failure. As Care Failure from Die Mannequin might say, “R.I.P. Caroline Kawa. I’d seen them around town, and Regina was the obvious choice for the location of Hard Core logo 2’s filming. She was a great person in every way. Too suddenly taken from us,” he lamented.
